The formula

Concrete yardage is just volume converted into cubic yards. Multiply the three dimensions in feet for cubic feet, then divide by 27, because a cubic yard is 3 × 3 × 3 = 27 cubic feet:

Cubic yards = (Length ft × Width ft × Depth ft) ÷ 27

Worked example. A 20 ft × 12 ft pad at 5 inches deep: 5 ÷ 12 = 0.417 ft, so 20 × 12 × 0.417 = 100 cubic feet. Divide by 27 to get 3.70 cubic yards. With 10% waste you would order about 4.07 cubic yards.
